Population Health Management Tools | Practice Fusion

We’re excited to announce the availability of population health management — a new platform within your EHR to help you use data to improve your patients’ health.

Practice Fusion’s population health management platform builds off features that you may already be familiar with in your EHR, including dashboards and clinical decision support notifications also implemented as part of Practice Fusion’s 2014 EHR certification. The platform is designed to make it easier for you to track and manage groups of patients across different clinical measures. Each clinical program begins with the identification of patients who are at risk or are missing recommended tests or treatments.

You’ll see population health management throughout your EHR in the following locations:

  • Summary and detailed dashboards that allow you to track your efforts to improve outcomes and quality based on clinical guidelines
  • Clinical decision support notifications at the point of care to alert you that you are seeing a patient who may need attention
  • And coming soon, optional opt-in patient communication tools to help you engage patients in their care when they are outside of your office

The first step: adult vaccinations

This Friday, we are launching a series of features to help you increase vaccinations rates for adult patients eligible for specific vaccines according to Centers for Disease Control (CDC) recommendations.

We have created a set of dashboards that provide a daily snapshot of your patient population at the aggregate level and at the individual vaccine level. You will also be able to see your current rates of vaccination compared with the average rates of vaccination for all Practice Fusion physicians, displayed at an aggregate level.

To help you identify which of your patients haven’t received the recommended vaccinations, clinical decision support notifications will be displayed in the new chart note for eligible individuals.

In the coming weeks, we will introduce tools to let you automatically reach out to your patients before or after a visit, to help you engage with your patients when they are outside of your office. These tools will be made available on an opt-in basis.

The longer view

In the long term, Practice Fusion’s population health management platform will help physicians manage a broad range of diseases and conditions, as well as improve patient engagement and other outcomes that may be included in various quality improvement initiatives. You can expect new clinical areas such as BMI/weight management to be added to the platform in the coming months.

Data and technology can make your monitoring quality performance and patient engagement easier by automating routine tasks, and can help you improve the care of your patients without increasing workload. We are very excited for this initial launch, but improving rates of vaccination among patients on our platform is just the beginning.
